Is It Really Tolerance?

When ADHD medication feels less effective, tolerance is often blamed - but it's rarely the only factor. True pharmacological tolerance does occur, but many other variables can mimic tolerance symptoms.

Before assuming tolerance, consider: Has your sleep quality changed? Are you more stressed? Have you changed your diet or eating schedule? Is your medication being taken consistently?

Common Reasons Medication Effectiveness Decreases

Sleep deprivation: Poor sleep dramatically reduces stimulant effectiveness and worsens ADHD symptoms independently.

Increased stress: Stress depletes neurotransmitters and increases cognitive load, making medication less sufficient.

Hormonal changes: Menstrual cycles, pregnancy, menopause, and thyroid issues all affect medication response.

Dietary factors: Eating acidic foods or vitamin C near medication can reduce absorption. Protein intake affects response.

Medication inconsistency: Taking medication at different times affects blood levels and perceived effectiveness.

True Tolerance vs. Pseudotolerance

True tolerance involves neurological adaptation to the medication, requiring higher doses for the same effect. This develops gradually over months or years.

Pseudotolerance is much more common - external factors make medication seem less effective, but the underlying response is unchanged. Fixing the external factor restores effectiveness.

What To Do

Track potential variables: Sleep, stress, diet, timing, and symptoms. Look for patterns.

Discuss with your provider before assuming you need a higher dose.

Consider a medication holiday (only under medical supervision) to help distinguish tolerance from other factors.
